Rollover Checklist: Three Steps to Roll Over Your 401(k)

Note: If you are transferring an IRA from another financial institution, please use the IRA checklist.
First, you’ll need to choose the appropriate IRA to open. Or, if you’ve already opened an appropriate Fidelity IRA, you can skip to Step 2. IRA rollover options include:
Fidelity Rollover IRA – Eligible distributions from a 401(k) can be rolled over directly without incurring any tax penalties and assets remain invested tax-deferred.
Fidelity Roth IRA – Eligible distributions from a 401(k) can be rolled over directly; however, taxes will be incurred. You can also roll a Designated Roth Account, also known as a Roth 401(k), into a Roth IRA. To learn more, call a rollover specialist at 800-343-3548.
Fidelity Traditional IRA – You may also roll your 401(k) assets into an existing Fidelity Traditional IRA; however, note that you may not be able to convert pre-tax 401(k) funds back into an employer-sponsored retirement plan. To learn more, call a rollover specialist at 800-343-3548.
Request a direct rollover from your old 401(k) plan. A rollover specialist can help you contact your former employer's plan administrator, or if you prefer to contact them on your own, try our online processLog In Required.
- Complete any forms required by the plan administrator.
- Instruct them to make the check payable to: Fidelity Management Trust Company (or FMTC), FBO: [your name]. Be sure your IRA account number is included on the check.

- Deposit the check with Fidelity in one of the following ways:
- You can also have your former employer wire your funds to Fidelity. View information needed to wire to Fidelity
- Fidelity will notify you by email and by U.S. mail when your funds have arrived.
